Knee Fractures What are they?: A knee fracture can be a break of any of the bones around the knee–Femur (thigh bone), Patella (kneecap), and Tibia (shin bone).
 Important Facts: There are many causes of knee fractures. A very common cause is a car accident. A blow to the knee during almost any contact sport can also result in a fracture of the knee. Falls from heights or even sudden indirect forces, such as stumbling down stairs, can cause knee fractures.
 Statistics: Patellar fractures account for about 1% of all fractures. They are most common in people who are 20 to 50 years old. Men are twice as likely as women to fracture the kneecap.
 Treatment Duration: Treatment for knee fractures is based on the severity of the fracture and the bone involved. Usually fractures of the femur require some form of surgical stabilization. Mild knee fractures of the patella can sometimes be treated with immobilization. Displaced patellar fractures, however, require surgery. Finally, tibia fractures are highly variable and need individual assessment prior to developing a treatment plan. Most surgery involves the placement of screws, plates, wires or rods.
Do’s/Don’ts: Apply a sterile dressing to open wounds. Don’t move the patient unless the injured area is completely immobilized. Don’t attempt to straighten a misshapen bone or joint or to change its position. Don’t test a misshapen bone or joint for loss of function.
Common Myths: Myth 1: Severe intra-articular fractures should be treated in minimal fashion to leave bone stock for later surgery. In tibial plateau fractures, a common expectation is that the patient will ultimately need a total knee arthroplasty (TKA), so the injury is addressed with a midline incision and double plating. “This is bad thinking that will lead to bad results,” Dr. Ostrum said. In one series of almost 14,000 TKAs, just 0.0045 percent was subsequent to tibial plateau fractures.

Pediatric Knee Pain: Oschgood Schlatter’s Disease

Pediatric Knee PainWhat is it: Oschgood Schlatters disease is a very common cause of pediatric knee pain, especially in young athletes, usually between the ages of 10 and 15. It occurs due to a period of rapid growth, combined with a high level of sporting activity. Though it’s one of the most common causes of pediatric knee pain, it’s really not a disease, but an overuse injury. OSD can be quite painful, but usually resolves itself within 12 to 24 months.
Important Facts: These changes result in a pulling force from the patella tendon, onto the tibial tuberosity (bony protrusion at the top of the shin). This area then becomes inflamed, painful and swollen. This is frequent in younger people due to the remaining softness in their bones (from childhood). Pediatric knee pain from OSD usually strikes around the beginning of their growth spurts, the approximately 2-year period during which they grow most rapidly. Growth spurts can begin any time between the ages of 8 and 13 for girls, or 10 and 15 for boys. OSD has been more common in boys, but as more girls participate in sports, this is changing.
Statistics: OSD usually causes pediatric knee pain in children and adolescents ages 11 to 15. One study suggests that 13% of all athletes suffer from OSD.
Treatment Duration: Pediatric knee pain caused by OSD can be quite painful, but usually resolves itself within 12 to 24 months.
Do’s/Don’ts: This injury needs rest if it is to heal properly. Only do as much exercise as it will allow without causing pain. Weight bearing exercise will make the knee pain worse. Keep sessions few and high quality rather than train every day. Apply ice or cold therapy to the knee regularly throughout the day to reduce pain and inflammation and particularly following activity or sport. Ice should be applied at least three times a day for 10 to 15 minutes. Do not get steroid injections as this may cause weakening of the infrapatellar ligament, scaring and fat necrosis.

Pilates: What Exactly is it and How Can it Benefit You?

Pilates has become a popular form of exercise since it was first developed in the early 1900′s.  It can be done on a “Pilates machine” or “Reformer,” but the preferred method, and the one that is best for beginners, is often Mat Pilates. This is much more accessible as it requires no equipment and can be done just about anywhere. A Mat Pilates class offers the same benefits as a Pilates machine does without the hassle, simply by using your own body weight to build strength. Additionally, Mat Pilates is ideal for beginners in order to learn how to do the movements with proper form.
Unlike many other forms of exercise, Pilates requires a certain amount of mindfulness, focus, and awareness of the body. For this reason, Pilates is a fantastic way to tone and condition the body with simple yet challenging movements. Pilates teaches us to engage the proper muscles in order to get the most out of a workout and to protect ourselves from injury. Pilates is all about using the “Powerhouse” of the body – comprised of the abdomen, back, hips, and thighs. Pilates not only works on strengthening the core but also stabilizing it. Developing core stability is critical for building a solid foundation for other workouts as well as activities of daily life. Without a stable core, our bodies are more prone to injury and pain.
That being said, it is important to be sure you are performing Pilates exercises correctly – improper form can do more harm than good. Before going out and buying a Pilates DVD or unrolling your mat in the back of a large class at the gym, make sure you have set a good foundation in Pilates form and technique. A good way to do this is to find a small class where you will be able to get personalized feedback from the instructor. Total Hip Replacements: What You Need to Know

What is it: Total hip replacement surgery, also called total hip arthroplasty, involves removing a diseased hip joint and replacing it with an artificial joint, called a prosthesis
Important Facts: Hip prostheses used in total hip replacements consist of a ball component, made of metal or ceramic, and a socket, which has an insert or liner made of plastic, ceramic or metal. The implants used in hip replacement are biocompatible — meaning they’re designed to be accepted by your body — and they are made to resist corrosion, degradation and wear.
Total Hip ReplacementStatistics: Between 200,000 and 300,000 total hip replacement operations are performed in the United States each year, most of them in patients over the age of 60. According to the American Academy of Orthopedic Surgeons (AAOS), only 5–10% of total hip replacements as of 2002 were in patients younger than 50.
Treatment Duration: A standard total hip replacement operation takes 1-1/2–3 hours. Most patients can start driving six to eight weeks after the operation and return to work full time after eight to 10 weeks, depending on the amount and type of physical exertion their jobs requires.
Do’s/Don’ts: DO NOT move your operated hip toward your chest (flexion) any more than in an right angle. This is 90 degrees. DO NOT sit on chairs without arms. DO NOT sit low on toilet or chair. DO NOT turn your knee cap inward when sitting, standing, or lying down.
Common Myths: “There’s a new surgical technique or approach where the patient has total hip replacement surgery, throws away the crutches and goes home the next day”
Sounds too good to be true? It probably is. Of course, it is possible to have total hip replacement surgery, take a significant dose of pain medication and go home the next day. The reality is that total hip replacement is a major operation involving trauma to tissue and removal and replacement of bone. In the interests of your best recovery, there has to be a period of recuperation and rehabilitation.

Hip Avascular Necrosis: Facts and Myths You Need to Know

Hip Avascular Necrosis

What is it: Hip avascular necrosis (commonly called “AVN”), now better known as osteonecrosis (“osteo” means bone; “necrosis” means death), results from interruption of the normal blood flow to the femoral head, or the “ball” that fits into the hip’s socket. If left untreated, this loss of blood flow to the hip ultimately causes the “ball” to die and collapse, leading to extreme pain, loss of hip motion, and eventually, severe arthritis.
Important Facts: There are two major forms of hip avascular necrosis: traumatic or non-traumatic (meaning no injury). Hip fractures or hip joint dislocations, partial or complete, are the most common causes of hip avascular necrosis. The most common risk factor is the use of oral or IV steroid medications (corticosteroids such as Prednisone). Another very common risk factor is excessive use of alcohol: the greater the consumption of alcohol, the higher the risk of developing avascular necrosis.
Statistics: In fact, about 25% of patients with hip avascular necrosis develop the disorder for no apparent reason. About two-thirds of the people who Hip Avascular Necrosisdevelop hip avascular necrosis are in their 20′s and 30′s. Avascular necrosis is diagnosed in about 20,000 people each year.
Treatment Duration: Once the hip joint is severely damaged (severe collapse and arthritis), then hip replacement essentially becomes the only reasonable treatment option. If hip avascular necrosis is diagnosed in its earliest stages and the diseased area of the femoral head is very small, a procedure called core decompression and bone grafting can be done with some success.
Fortunately, there is good news for adolescents and young adults under the age of 50 diagnosed with hip avascular necrosis, including select patients with later stages of disease, larger sized lesions, and limited collapse. A highly specialized surgical procedure called free vascularized fibular grafting – a unique form of bone grafting – can restore the dying hip and prevent hip replacement with much greater success.
Do’s/Don’ts: Pain in the hip? Don’t ignore it.
Common Myths: Most young adults do not think about themselves getting ill or let alone having “arthritis” – a rite of passage envied by anyone over 40 years of age. However, this youthful mindset can delay its diagnosis. Diagnosing hip avascular necrosis can be very difficult, and what makes it most elusive is its ability to exist “silently.”
